I would probably classify the second baby you describe - the one who only 
nurses in her sleep - as a nursing strike. She's learned to associate 
feeding with pain, from your description, so when she's awake and conscious 
she resists it. When she's asleep and less aware her baby instincts kick 
back in and she nurses. This is a common way of dealing with a nursing 
strike, and I would expect that over time, as the baby feels better and 
realizes that nursing no longer causes pain, the mother will be able to 
breastfeed her while she's awake too.
