Hi everyone-
I'm a PNP/lactation specialist (studying and gaining hours to sit the
exam :) ) in a group private practice and am working with a baby who
has me stumped. In brief:
2mon old male who has breastfed beautifully from birth
3 weeks ago, mom was ill and on amoxicillin for bronchitis and dad
gave bottle of formula so mom could "rest". Babe still woke up 2 hrs
later to nurse and mom noticed hives on face. Babe never in resp
distress or even remotely uncomfortable. Parents took baby to ED where
babe received IM dose of benadryl and epi. At ED, mom was told baby
had a dairly intolerence and to immediately stop all dairy products in
her diet because "the next time the reaction could be severe". Babe
nursed in ED before going home (of course, mom had had dairy in
previous 24hrs and theoretically was still floating around in her
system...)

Babe has continued to thrive, gained great weight- 22 oz in 11 days.
Actually has made me think mom has a bit of oversupply and that's what
is causing some GI distress. About 1 week ago, babe started having 1-3
flecks of blood in stool every 4-5 days . Stools also vary between
green and pale yellow and usually watery. Stools are still occurring
every nursing session (every 2-4hrs). Mom brought baby in to my office
and I examined him, no tears/anal fissures. Baby is happy, not colicky
and again, growing well. Mom has now basically eliminated corn, wheat,
dairy, soy, eggs, citrus and caffeine from her diet. She is very
concerned about this blood and even though I have reassured her that
in light of how her baby is thriving, it's not dangerous, she now
wants a peds GI referral. I agreed reluctantly, because our peds GI
guy isn't very supportive of BF and almost always switches BF moms to
formula. This mother is very committed to BF  and I don't think she'll
give it up, but any help you all could provide would be great.
