I function as the LC for a very large level III NICU in VA. In the past
year a policy has been instituted where all colostrum/milk from a mother of
a baby under 32 weeks gestation is frozen for 3 days before it can be used.
This was based on the info in Lawrence pg582 (5th edition) There was talk
initially of testing the mom, but the cost was too big a problem. There are
a few of the docs who if a mom is known to be CMV negative will let the milk
be used without the 3 day freeze, but that almost never happens. (About a
month before that policy came about we did have one baby who was in that
range who developed a CMV infection after getting mom's milk and was very
sick.) I was not in favor of this policy due to all the known benefits from
fresh milk. We have a special handout explaining the policy to parents.
