I notice that many preterm infants are used to being fed, and not used 
to feeding. Pre-filling the nipple shield with milk (a curved 
tip/periodontal syringe tip  fits right into the holes in the teat of 
the shield) might stimulate a swallow and allow the baby to get into a 
suck:swallow rhythm. Dad is the ideal person to keep filling the shield 
as the baby learns. Once the baby sucks correctly, the breast will 
release milk and the pre-filling is no longer necessary.

Perhaps I'll put this trick up as my next "clinical corner" tip on my 
website.
