I know it's old ground, but what about working on a release form laying out the risks?  *Just the ones in the AAP policy statement.*  Why is that such a hard step for a hospital to take?  The AAP has already taken it in publishing its statement.  Heck, maybe just copy a few key parts of it word for word and have parents sign that they've read it.  Would a health facility truly balk at such a move?  With what justification?  

I think one reason is it would make them look awfully *foolish*  to have parents sign a consent form to use a product that the hospital staff are giving samples of/promoting the use of as parents head out the door on discharge.  Hospitals don't want to give up those handy dandy little "gift" bags so they sure don't want to face the fact that formula use carries risk.
