Brenda:  I have encountered this situation almost exactly and had success in
helping the mom bf.  She was having awful supply problems and was told by
another LC that "she'd NEVER have enough milk and that she should just give
up and use formula."  Argh is all I have to say.  The mom was devastated,
but persevered and called me at the prompting  of a friend.  I have this
thing when someone says "you'll never..."; apparently so did this mom.
Anyway, we did all the usual: lots of nursing, skin to skin, put mommy and
baby to bed, taught mom infant massage (to boost her prolactin and baby's wt
gain), used Fenugreek and Marshmallow, NSP's Distress Remedy, and what
finally topped the protocol off with Vitex (chaste tree).  The whole routine
was only having moderate success for a couple of weeks (but enough success
so that we still weren't supplementing with formula) until we added high
dose vitex (working up to 4 450 mg capsules, 3 times a day).  I'm happy to
say that this mom is now 51 and still nursing her 2 year old (and never did
have to give a drop of formula).  I hope this helps.
