We have a phone log to record each call.  We include date of the call,
mother's name, delivery date, doctor, reason for call.  It is very brief.

We also have a full page form to record all the details -- why the mother
called, baby's stats, what the mother says and what we recommend, whether
we referred and to whom.  That goes into the mother's folder, which is kept
in the LC office.  We organize folders according to delivery date.
