"Phone calls are constant
over a 6 day work week. Does anyone have a solution on how to keep
records in an organized manner."

We just kept a notebook by each phone usually used to take these calls,
divided into columns for date, issues discussed, rec. given, and if any
further action (call back, for example, or home visit, or referral to
someone else) was necessary. Didn't usually add notes to discharged pt's
chart, but if would if some really extraordinary measures were taken & I
felt that it should be part of that pt's record. It really was not a big
problem - well, no more of a pain than any "paperwork" you're trying to
squish into an already over-busy day is. Plus it was useful in protecting us
from complaints of "but when I called last week, the other lady said...";
and we could use the log in quarterly reports - it always looks impressive
to have documentation of all that stuff that you're doing already anyway.

Hardest part was getting all staff to doc. in the log. As always. Try to
keep it as simple as possible.
