Heather, I have a positive story from a while back to share.  Mother was
told she couldn't breastfeed her first two babies because of inverted
nipples so didn't try.  16 years later she had another baby.  She was
well-prepared, decided to wear breast shells/nipple formers and even
insisted on wearing them in theatre during her Caesarean.  She spent the
first few days in frustration and tears until she and baby got it worked
out.  Baby received no bottles and didn't use a pacifier at mom's
insistence.  Her baby blossomed and so did mom!  She breastfed that child
for 5 years and after all that time her nipples had still never seen the
light of day!

The major obstacles I think are lack of support and skills and impatience.
Often here,
even mothers with flat nipples are told their nipples are not good enough
for breastfeeding.
