I had a friend who successfully nursed first baby and produces a lot of milk. She did have one bought of mastitis with her first child.  With her second she had those symptoms and we could feel a lump and see a large white spot on the nipple.  I do deep breast massage and worked on her  three separate times  for about an hour each time with some relief.  She tried using a sterile needle to release the plug to no avail. Finally took her to my husband who is an OB/gyn who actually numbed her up and  cut off a small portion of the skin over the plug.  After she nursed, the plug broke and the lump resolved.  She did some internet searching and found that this problem is very rare - to the extent that surgery is needed.  It did not reoccur.
