Way back on reading again, so maybe this has already been adressed, but just wanted to add my thoughts anyway.
I do not see how it will be harmfull to parents or their baby to encourage them to question the advice their doc gave them when this is really way off evidence based practicve and maybe even harmfull. I, indeed, feel it to be my ehtical duty to do so.
Adding powderd formula to human milk in an attempt to ake it mor nutritious is not a really good idea for various reasons. It will easily canotaminat mom's milk, which especially for a vulnabl child is not good. If it ony is to increase calorie intake by the child their ar lots of ways mom can increase her milksupply, increase her childs'intake of her milk and to increas the average calory count of her milk. Adding other things is most often not needed at all. If anything else above her own milk is neded using human milk is always th first route to take.

Warmly,

gonneke, IBCLC in PP. LC lecturr in christmas-card-snowy southern Netherlands

I have not been part of this discussion, but wanted to give you a "heads up" that:
1. we should NOT tell parents to go against MD instructions - we can encourage them to call to clarify the instructions, and explain our concerns.
2. I know personally of VERY supportive MD's who instruct parents to mix formula with breast milk for babies who are being discharged from NICU and need the extra calories (usually this is done for SOME not all the feeds, as breastfeeding is still encouraged).
3. The cost of the breast milk fortifiers, along with their unavailability (a mom with twins called me in a panic after driving for over 3 hours trying to find it) has led many doctors to use this method to increase caloric intake.
