Liz, I have a lot of these long termers in my practice. We do need to be
realistic and help moms return to normal(whatever that means). I have found
it helpful to set short goals and I encourage as much help as possible. This
means bottle feeding with formula to bridge the gap in supply.Long term use
of cups, tubes etc often is just too much. I encourage moms to find that
place that works for them without destroying the whole familly structure.
Mothers still have to care for other children etc. THis means that guilt
over what we can not control goes out the window. I stress that a little
breastmilk is still good or some time at the breast is great etc.Guilt is a
waste of time and I tell mothers this. In order to continue long term, the
plan can not consume every sleeping and waking moment. Motherhood can not be
measured in ounces of milk and many fantastic mothers do no have a full or
even close to full supply. It is extremly rewarding to hear from mothers one
year later that, despite not being able to fully supply baby's milk, mother
is still doing some breastfeeding, loving it and feeling pride in her
ability to mother. One of these mothers gave me the article from Baby Talk
about Breast and Bottlefeeding. We had an interesting discussion about what
helped her to continue despite many problems.  A supportive and non
judgemental attitude, which I know you to have Liz was the 2 top things for
this woman. These hard cases are sometimes the most rewarding after being
