with mom's permission, i am posting her situation to lactnet, in hopes that
someone will have a pearl of wisdom we haven't tried yet.

Mom had emergency c section, first baby, gestational diabetes, cracked
bleeding nipples due to poor latch (now corrected), antibiotics 2 X in
first 4 weeks.  Diagnosed with thrush (surprise surprise!) and treated with
nystatin, then baby and mom treated simultaneously (don't ask!), NO
improvement, while mom followed hygiene and most dietary protocols for
thrush.  Nystatin followed with Lotrimin, followed with Diflucan (400 mg.
loading, 200 mg daily X 14 days).  Deep breast pain went away after
diflucan, but severe, excrutiating nipple pain persisted.  mom can't bear
to pump more than once per day (more painful than nursing),. was limiting
nursing time, baby's weight faltered, supplementing with SNS, baby taking
3-4 oz formula, 6 times per day by 10 weeks.  baby now 12 weeks old. Mom
tried gentian violet, all NO improvement.  Tried (and continues to use)
Jack's APNO  - one side got MUCH better for 2 days, then worse again.  Mom
got her milk and nipple tissue cultured (i know its inconclusive, but still
.. .)  - negative for staph and yeast.

A few more tidbits of info:  her early pregnancy nipple pain never went
away, she does not have visible signs of raynauds, her nipple looks exactly
the same going in baby's mouth as it does coming out, her blood glucose
levels are normal (per home finger stick tests), the baby appears to have a
good latch, and a good suck - tongue down, frenulum normal.  Pain is at
latch on, during feeds and between feeds (can't stand water to hit her
breast in the shower).  Only other, untried thoughts are cranio-sacral
therapy if baby is uncomfortable from birth and somehow clenching, or
something - but as i said - no visible signs of trauma to the nipple.  Mom
has seen hospital based IBCLC, who treated fro thrush, but ahs no further
ideas.  ANY and all thoughts or pearls desperately needed and encouraged.
COuld this still be yeast?  No shiny, flakiness, all the treatments, NO
impreovement .. i know it can be persistent , but shouldn't she notice some
improvement with at least one of the various treatments?  and why would the
APNO seem to make things better on one side for only two days?  thanks in
advance for your help.  as you cna imagine, this mom is about to give up.
it is amazing she made it this far!
