Hi, I have been an LC working in a hospital in Central New Jersey for three
years now. I usually am a lurker but today I have a question. I have a mom
who just had her second baby. She quit breastfeeding with the first due to
depression while breastfeeding. She would like to make a go of it this time
but has started to get the same feelings all over again. She has a feeling of
nausea and sadness while she is breastfeeding. I can assume that this is
related to the releases of oxytocin during the MER but I'm not sure how I can
help her with this situation other than reassure her that this can be
perfectly normal. If anyone has had experience with this situation or can
help me counsel her please help.

I don't think that it is safe to assume that her feelings of sadness are due
to oxytocin release. You should consider perhaps that this woman is dealing
with some other issues, perhaps even sexual abuse issues, that cause tactile
memories to be triggered when that rush of sensation through the breast occurs
at let-down. I would try to talk to her about whether she has other issues
that may need to be dealt with by a mental health professional to help her
deal with these feelings while she nurses (and also how they may be affecting
other aspects of her life). Penny Simkin gave several wonderful talks on this
subject at recent Midwifery Today conferences which I have heard on tape. The
tapes are for sale and you can get more info at the MT website. I highly
recommending buying and listening to them (and frankly anything Penny has to
say about anything).

Good luck.
