Some time ago I posted about a 46 year old first time mom who was pregnant
by "embryo adoption."  She delivered by c-section, and I have been following
the babe since.  Baby was born at 6 lbs. 8 oz, and nursed great right from
birth.  She was discharged on day 4 at 5 obs 15 oz.  She returned for follow
up on day 8 and was 6 lbs 2 oz, so we thought all was going well.  However,
1 week later she was down to 5 lbs 14 oz.  Baby was nursing "all the time"
and was stooling and wetting adequately.  Mom started to pump post feed
several times a day and supplement with this pumped milk.  Baby was still
slow to gain, was 6 lbs 2 oz after a week of this (4 oz gain, baby at that
point age 3 weeks), but mom was reluctant to do anything else, and baby was
vigorous and healthy appearing.  Baby has been mostly exclusively
breastfeeding since then, although mom pumps usually once a day to encourage
supply, and sometimes supplements that milk.  Baby at 4 weeks was 6 lbs 8 oz
(birth weight) and today at age 5 weeks was 6 lbs 15 oz.  So it looks like
supply has finally picked up and baby is doing well.
Not sure if the slow start was due to usual factors (c-section, first time
mom, small baby) or due to mom's age or what, but it seems to be getting
better with just a little help.  We'll continue to follow closely for a
while longer.
By the way, this mom was very grateful for breastfeeding assistance and has
made me feel really glad to be able to provide it.  She was very nervous
that something would go wrong, then had a very difficult c-section and rough
recovery.  I brought the baby to her at about 1 1/2 hours of age, helped
them get skin to skin, and assisted her with first feeding which went great.
 She's still convinced I did something to the baby to make her nurse well!
