Can honestly say I have never heard of one forming.  I've been doing 
lactaion stuff since 1967 and have been  to lots of conferences.  I would 
think being bathed in breastmilk (via leaking ) would be very healing.

Counseling the Nursing Mother (2005 ed) mentions one warning  (Schackmuth, 
"Milk fistula: A complication after core breast biopsy." Am J Roentgenol 
161:961-962; 1993.

Lawrence, Riordan and BFAB don't have anything in their indexes.
