Yes, it is possible to have unilateral thrush. I have found that it is
usually in a case where one nipple had experienced more trauma, which
invited an intrusion of yeast into the tissue.

Early on, I would assume that it was not thrush if it was in only one
nipple/breast, but I have seen it too many times to stick with that
assumption. In those cases anti-fungal measures resolve the problem.

There are on-going surprises with the yeast question. I had a mother who
had no sore nipples at all but whose baby was very fussy. While exploring
the cause, she looked into his mouth and it was totally covered with white
globs of yeast--tongue, lips, cheeks--all covered. As soon as she began
treating his thrush, he began feeling better.

So one can have a client whose baby has no visible symptoms while she has
symptoms of yeast on nipple and breast, and one can have a client whose
baby has a raging case of thrush while she has no symptoms. I do recommend
treating a baby who has thrush even though mother is not suffering. Baby
is. His throat is sore and his mouth feels like a vagina feels when a woman
has a vaginal yeast infection. And usually his diaper area is hurting too.
