I can't answer your last question, but can tell you that in my (one)
experience, I leaked from about 4-5 months along (enough that I had to
wear breastpads) and delivered exactly on my due date.  I did have a
very very abundant supply though, and my guess is that the PT leaking
was an "early red flag" that I didn't know how to decipher.
