I do agree that many, if not most, moms need a larger flange size. I have
Ameda products at work, and have to buy the larger flanges out of my grant
budget, which will soon run out. I am hopeful that Ameda will go to the
larger sizes as standard soon. I think it would be easiest and most
economical to just have the large size and use the inserts to size down if
needed. If it was not a money issue, I would definitely start all moms on
the larger size. If it is obvious that the nipples are larger, I start with
the large. But if not, I do explain to the mom how it should fit, and I
observe and have her observe if over time she needs the bigger size. If it's
a mom of a premie baby, I give the Barbara Wilson-Clay and Kay Hoover
handout with photos about pumping for a premie. It includes a photo of a
good fit and a too tight fit for the mom to see. (no financial interest in
this handout).
