I think this is a case where a nipple shield could help. Not as a magic
bullet, but something to try when baby is feeding well. I actually would
suggest bottle feeding (paced feeding, in a bf friendly manner). Important
components of the plan would be:
Protect the milk supply with regular frequent expressing, hands on with a
hospital-grade pump, if financially feasible.
Kangaroo care. Bottle feeding. Try transitioning to breast with nipple
shield, possibly after infant has taken 1/4 to 1/2 of his usual feed by
bottle. Emotional support.
