I got carried away on a rant.  I do know that 82 lines is not a short post.  
Even so, I forgot to mention that I have seen *rare* cases in which milk 
production in women with no history of breast surgery did not respond to 
frequent stimulation with careful attention to every single point in the process 
and every imaginable effort on the part of the woman, and I have concluded 
that there must be an organic cause for the lack of abundant, or even 
sufficient, milk.  In a minority of these cases there has been a known medical 
condition which can affect milk production, including a case of diabetes 
insipidus triggered in labor.  I have also known women who have had breast 
reduction surgery, hypothyroidism, and PCOS all at once, who have exclusively 
breastfed.  I know plenty of diabetic women who have exclusively breastfed, 
and most of the IVF mothers we see do so too.  
We only have 2000 births every year in my hospital, but a high proportion of 
mothers who have problems with breastfeeding do come through our clinic.  I 
am always open for the possibility that their problem is organic, but in my part 
of the world it is not common.
I don't think it is so far-fetched to think that this might differ according to 
environmental and dietary influences.  Norway is very restrictive on the use of 
antibiotics in animal feed and hormones in animal feed are forbidden.  But the 
potential to improve outcomes by the simple measure of practicing in a baby-
friendly way, is huge.
