From what you've described, I suspect that this mother may not be able to
make enough milk for one baby, let alone 5. In discussing "insufficient
mammary tissue", Chele Marmet advises to pay special attention to mothers
who have had reduction or aumentation surgery on one side only. Such major
lopsidednes of the breasts can be a red flag when it comes to milk supply
problems. In addition, you have described a "cone shape", which Chele warns
may be less elastic than usual. Between all of these, you have some major
causes for concern.

Of course, the proof is always in the pudding. I would certainly get mom
pumping stat [would probably teach breast massage and compression to go
with] after birth and just wait and see what happens. If supply is slow to
come, you can consider herbal galactogogues, and/or prescriptive ones.
However, it would be prudent to know that there may be a very low ceiling to
this mother's ability to make milk, so try to set some reasonable
expectations and goals. I'll be interested in hearing how this all turns
out!

BTW-- are these natural quints or the result of fertility meds? If the
latter, was there any dx?
