Have you tried pulling baby off center, nose lined up with nipple, then
tipping baby's head back slightly to lead with jaw for latch?  Diane
Wiessinger's article in JHL has excellent pictures showing how planting
the lower jaw first can help get the mouth wider open.

Another thought:  does the baby have a tight labial frenum that makes
flanging uncomfortable?
