Dear Elaine, We were having lots of problems with breastfed babies being
given formula feeds overnight by overzealous midwives. Our solution ( us
being myself and two other L.C.'s employed at the hospital) was to develop
a complementary feed consent form, we utilised the BFHI information for
medical indications for complentary feeds, and made it that any formula
given to a breastfed baby had to be ordered by a medical officer and
he/she had to complete the form, indicate the reason for the feed, and
sign it. The midwife and mother then also had to sign it.
 We purposely made the form long and complex( therefore a nuisance to fill
out - much easier to just help the mum with a breastfeed), requiring the
midwife to sign it twice, and you can imagine how happy the medical
officers are to be called to complete a consent form for a formula feed.
 Some mothers still request the feed, but at least we know it has been
discussed with them, the sheet contains specific information that has to
be discussed about how formula feeds are detrimental to breastfeeding.
