Hi All,
Just when I think I have a really good understanding of MER I get  
confused!
I thought lactogenesis 1 happened as a response to the placenta being  
released from the uterine wall. Pumping or not, BFing or not, the  
milk will come in, it just won't STAY in without demand. Am I totally  
off base here? Without pumping q3h in the first days PP, will we have  
missed the chance to establish a FULL milk supply?
Sometimes when I see a one or two day old who is alert when awake but  
very sleepy from a long 'abused' (from induction on, we can all fill  
in the blanks) delivery, and the mom wants to and is able to sleep, I  
encourage her to do so as I know what is going to happen when the  
drugs wear off and the baby comes to. Plus, it gives the worst of the  
edema a chance to subside in the areolar tissue instead of yanking it  
down with a pump q3h. This is not a baby with an elevated bili,  
lethargy or any other red flags. An otherwise full term healthy baby.  
Am I messing up??
