Hello friends,
Just thought I'd give a f-up from my call to Pharmacia about their product. I
made this same call 8 yrs ago when i started the lc job. There is no new
data. Their insert reads differently though now than 8 yrs ago. The emphasis
is on waiting 6 weeks so that the newborn isn't dosed a lot with depo via
milk (vs may inhibit lacation). The man i spoke with said that in some of
those early studies, clinicians didn't wait until 6 weeks for b/f but there
were no adverse effects on the kids, followed now to close to puberty.
However, no developmental delays said to him that they were receiving enough
nutrition. Unfortunately, i could make the same conclusion since it was not
clear when the depo was given (2 days pp or 4 wks are both earlier than the 6
wk reco) and it was not clear how long the moms exclusively breastfed.  He
also mentioned that their new injection that you receive once a month has the
same recommendation, strongly insisted upon by the FDA, that excl b/f wait 6
weeks and all other may receive within the first 5 days. Bottom line though
they support the clinicians weighting of risks:benefits, even though this
isn't printed on the package insert. At our hospital, the moms get 2 home
visits if they breastfeed, I wonder if giving it at the 2nd visit, shortly
after the milk comes in, would make a difference to the sensitivity of
alveolar cells to prolactin.
