Look at research at Australian Breastfeeding Lab.  Where baby vacuum measured with strain gauge and milk volumes measured.

(non-existing?) storage capacity in the milk ducts and the sticking of milk fat to the duct walls, apparently denied by the same research(er) and even more wondering about this after yesterday's interesting story about the trick to get strings of milk out, strings that certainly come from the ducts and not from the alveoli, I would reckon...)

I must have missed something. I did not know that vacuum was a prerequisit 
for full milkejection. If so, how come moms can easily and often even better 
hand-express than pump?

If you believe the research on let-down which shows that if there is not 
adequate vacuum on the nipple, when the milk lets down, it 'backs up" into 
less full areas and is less available on the next let-down,
