As to the first mom, well, this is a very common scenario in the US. 
Moms with twins and 22 mos old babies do not have support to bf. And,
since we have a culture in which it is easy to rationalize how AIM is 
"the next best thing", "just as good" and so on, well, why not. Some 
babies I feel a sadness about--babies whom I later wished I had 
known things I do now--like asymmetric latch, posterior tongue-tie, even
about CST (which I did not know for my own daughter fifteen years ago). 
Those babies, I wish I had done better for. But, the babies whose moms
cannot find a place within our utterly unsupportive culture, cannot find
it within themselves to hear their own intuition, hear their babies call
to be in their arms and at their breasts, to be one in synchrony--those 
babies, I think I cannot help individually. They will become part of the
statistics that will one day not be denied--will one day prove that there 
really is no "choice" in infant feeding in the way that we have come to
think of. We all have our paths, our parts to play and the contrast 
between what is and what needs to be is important to mobilizing change. 
Those babies we help by making a more humane world.

As to the second mom, have you sent her the article by Dr. Coryllos and 
Cathy Watson-Genna about posterior tongue-tie? Have you educated her about 
the option of domperidone? All of my clients who use meds use domperidone
and herbs in combination, usually quite successfully. I also address 
nutrition with my clients. We often use various modalities to effect supply, 
including acupuncture, chiropractic (can make a huge difference if mom is 
out of alignment), CST, homeopathy, ayervedic herbs, etc. But, this mom 
needs to know that having the tongue clipped simply needs to be done. Have 
you made an effort to educate the ped? I would also send the article to him.
If he is BF friendly, he should be quite interested in a conversation with 
you about posterior tongue-tie. I will say that I go to great lengths to help
a mom when she runs up against ignorance in the medical community or she is 
really committed, but has just apparently run out of options. Oftentimes, 
I can arrange for the mom to speak to another mom who has had a similar 
problem and this can be very supportive. 

I guess what I am saying is that there are moms we can help and moms we cannot 
help. There is always the work to be done in society that makes our other 
efforts unnecessary. Both matter. AS does every effort you make.

Subject: dissapointment

Well I am truly disappointed and depressed.  I have seen two clients in the
last couple of weeks (I work part-time only, so these are ALL the clients I
have seen.)  One was a mother of twins with problems with milk supply; I
just talked with her on the phone and she "gave up" stating that she had no
time while she was pumping, feeding and attempting to spend time with the
other babe (22 mo old).  She also said she gave up b/c her milk was getting
watery!!!!!  

Now, I have a girl that had a totally un-medicated birth b/c she knew that
"her body could do it, b/c that's what it is supposed to do." She is now
grieving the loss of the BRF relationship b/c she has no milk.  Both of
these moms were on Reglan and the last one pumped with a hospital grade
pump.  She is getting only 1.5 ounces from both breasts. She is afraid to
take Fenugreek & blessed thistle along with the Reglan.  So even tho' she
knows what her body is supposed to do she is giving up.  She does not see a
Ped but uses a Family Practioner.  She says that he is very BRF friendly,
however, he refused to clip the frenulum or refer her b/c babe IS capable of
covering the bottom gum, most of the time!  She is using donated milk from
LLL.  That's the other thing; she HAS been attending LLL meetings during
most of her pregnancy.   I saw them at home twice, but now they are going to
OU b/c it's free! How could this be happening?  And what are your thoughts
on Reglan & Fenugreek & BT?
