I would interpret this to mean just what it says, "require and obtain
consent to share"  It doesn't say that the LC is required to share info with
the PCP, just obtain consent to share.

My consent form says that the mother gives permission to share info with
her/her baby's PCPs.  I tell her I may need to, depending on the situation.

I do fax a copy of the consult to the PCP if they referred the client to me
or if there is info that they need to know in order to provide good care.
(Such as low or no weight gain, poor suck, etc)  But if it is just a
straightforward fix the latch and heal the cracked nipples, I don't see any
point, except as public relations / good will.  I certainly don't feel any
obligation in those cases.

When I worked in the hospital, the consent form was worded in much the same
way and we did not routinely fax copies to the physicians (this being for
the outpatient visits -- the docs saw the inpatients consults on the
respective charts).
