Hope you received your FAX.  I do have a "Consent to Supplement Newborn
Infants" that I developed for the hospital years ago when I ran the LC
program.  I did write it up in the Journal of Human Lactation a few years
back if anyone needs a copy here in Naperville.  The lawyers use to tell me
it would never hold up in court, but I have to say it really informed many
mothers of the consequences of supplementation and take some accountability
along with the nurses giving the supplementation/complementation.
