Perhaps the breast tissue fills up the oral cavity too much, or also
obstructs the nose a bit, whereas the bottle does not.

It just may be necessary to bottle feed the ebm, until he is able to have
the obstruction of the adenoids relieved. The breast could be offered

to finish a feeding or for comfort.

A second ENT opinion could be considered. Also might there be any  medical
management in the meantime?
