Hello all,
I have PTP.  I am working with a mom who has very large breasts and also
large nipples.  The baby is two weeks old and has not been latching at
all.  She is pumping full-time while continuing to attempt to get him to
latch.  She has a lot of breast tissue, and if she tries to support her
breast or compress her breast or shape the breast towards his mouth at all,
the nipple retracts in the center.  She also has to use the pump on the
highest setting to get the milk to letdown and to get enough milk.  When
she pumps, she does get a stream of milk, but there is only one stream of
milk going into the flange.  It seemed like there was one other place where
only a drop at a time was coming out.  She has been pumping for 30 minutes
each time to get enough milk.  She did have a delay in her milk coming in,
but has been able to provide breastmilk exclusively by pumping.  She had a
cesarean birth.  She is producing just a little bit more than what baby is
eating each day.

Does anyone have any suggestions for the combination of both very large
breasts/nipples and inverting nipples?  They are not inverted normally,
only when she applies pressure.  They can be stimulated to protrude, and
the pump does do that.  However, they will quickly invert if the breast is
at all compressed in certain ways.
