Dear Jennifer and all others who took the time to reply,
It helps a lot to know I'm on the right track at least.  There's no history 
of PCOS and no hypoplasia.  Her breasts look great - except for these 
inverted nipples.  We are feeding with a medela SNS under the nipple shield 
at the breast.  When I went in today the mum *looked* like a natural 
feeding - even with the SNS.  Yesterday she looked quite awkward still. 
Baby is very strong and vigorous and sucking really well - if anyone can get 
this milk going he will.  The mother's breasts were full and warm but she 
had felt that over night they were softer after she just breastfed and he 
settled well after some feeds, but I'm not entirely convinced there is much 
milk transfer.  I'd love to see what is inside those breasts - I think that 
there must be an anomoly in the ducts that means milk cannot be released.  I 
would love to explore the option of oxcytocin nasal spray - is it available? 
In Australia?   I think there are levels of fear, and more so in the staff 
caring for her (including me) and it can be hard to keep a lid on that!  I 
know some where thinking "just put him on the bottle" but the mother is 
clear that she wants to feed for as long and as well as she can.  And she's 
doing a great job
