I actually had a fairly significant uterine prolapse after pushing out my
rather large 2nd child (shoulder dystocia home-birth). My midwife agreed
that my suppressed estrogen levels weren't helping pull my uterus back in (I
was tandem nursing), but suggested other options, such as increased and
varied Kegel exercises (not just the squeeze-release we are all familiar
with), a pessary weight (I think) inserted into the vagina, which would
require the muscles to work harder to bring the uterus back in, and as a
last resort a low-dose estrogen cream to use locally in the area.

As it turned out, time (and Kegels!) helped a lot. Now what remains is
actually a slight prolapse of the vaginal wall -- not my uterus, but the
anterior wall of my vagina never sprung back. It's not bothersome or
noticable except on an internal exam. I had low back pain early postpartum,
which has resolved itself.

Needless to say -- neither of my children weaned. Is there any way to
enlighten this doctor? I shudder at how uneducated about lactation some of
these medical professionals can be!
