Hi Nicola,
Lo and behold, right?!  Smart baby, lucky mom.
The Breastfeeding Answer Book has a very good section on relactation.
While you check it out, see mom and encourage her to let baby latch--a lot. 
Take it from there.
Other suggestions for mom:  Practice lots of skin to skin contact to keep 
window open for latch and feed.  Nose to nipple, asymmetric latch.  Pump to 
avoid engorgement and restimulate a milk supply.  Replace formula supplement 
with expressed milk, try paced bottle feeding to help transition if mom is 
not ready to go with an alternative method of feeding.  Keep a log on how 
many nursings and how much supplement.
