I bought some of Medela's new large size breast flanges recently, and they
have worked well for a twin-mom I'm working with who was getting very sore
pumping for her pre-term babies.  Her nipples would swell up during pumping,
and rub on the walls of the shaft of the standard size flange.  The new,
plastic, larger flanges fixed the prob.  The large glass flanges were so
expensive I loaned them and autoclaved between users.  The new plastic ones
are still kind of expensive, but not prohibitively so.
