    Has your client's milk ever been cultured for yeast?  I am inclined to agree with Barbara Wilson-Clay (I'm sorry to use your name in vain, Barbara, but your lecture at the LATCH conference really got me thinking) that yeast is largely overdiagnosed.  Some of the lactation literature I've read says that yeast cultures sometimes comes out with false negatives.  However, I spoke with the microbiologist at my hospital and a friend who is an infectious disease doctor and both say that yeast grows very well in a culture medium and false negatives are probably very rare.  I would hate to see your client spend much money on treatment she doesn't need, although I really don't know how much the breast milk or nipple skin culture would cost, and what is the best way to culture for yeast is it's deep breast pain?  I'll ask my friend about that.  I guess my point is that your client may not have yeast at all.  Jack Newman says that gentian violet is very effective for treating yeast, so it seems she would have at least some relief with the gentian violet.  What do the rest of you think?
