I always do a weight on the baby even if already done that day by pedi in
case I need to followup, I want a weigh from my own scale. (The scales are
rarely the same in the area--even room to room in the docs office can mean the
difference of a couple of ounces.) I don't always check for intake because in
reality one feeding does not measure intake overall. Yes, if we are trying to
assess transfer it would give an idea of that particular feeding's transfer, but
not necessarily the one before, or the one after. If I am concerned about
transfer I want to follow a min. of three feedings, but preferrably 24 hrs of
mom providing the pre/post weight checks. If the need for service is related to
nipple soreness, engorgement, etc. but baby is gaining well and milk transfer
is not an issue I do not assess intake at all because it places a focus where
it is not necessary and there is not intake issue so why create one for the
mother to worry about. Watching intake matters for some babies, but not all.
For many 'normal' issues output is an adequate measure of intake as is weight
checks over a period of a few days. However, I do believe that when an
accurate intake measure is needed, the little store scales don't cut it. One LC I
know says that when worrying about intake, the 1/2 ounce measure is fine because
either way the 1/2 ounce is not a big deal. I tend to disagree because I do
not want even 1/2 ounce more of supplement formula in a baby. Now, if it is
expressed milk, not such a big concern and it could be less of an issue to give
a bit more than needed. I do wish Medela was not the only source as I am a
bit put off by the lack of competition in the product. (Just my
free-enterprise capitalist mind at work---always seeking the less expensive but equal
quality. Haha.)
