Hi Jessica,
I have seen many marathon runners in my practice, and none have had any
problems with breastfeeding/milk acceptance after a 5-10 mile run, even
daily.  We have to beware of making blanket decisions from a single case
report.  Perhaps the mom in the case report by Duffy drank artificially
colored sports beverages during her run, and this is what the baby was
reacting to.  Perhaps waiting through the run and for an hour or two
afterward overfilled the breast and decreased the milk fat, giving the
baby a relative lactose overload. There could be a zillion possibilities.
As for the Wallace study, infants were given the milk in an eye dropper,
not by breastfeeding in an attempt at blinding the mothers as to whether
they were using post or pre exercise milk.  The exercise in this study
was done to exhaustion, most mothers will stop far short of that point.

Most moms have heard of these studies, and are concerned, but have no
problems once they are reassured by the differences between the
experimental conditions and their real life exercise regimens.  Usually
I just recommend that mothers breastfeed before exercising, to increase
breast comfort during the exercise.
