I think the biggest reason is that there was no economic interest in
sleeping position whereas there was for feeding method and not
smoking!  (Of course, some companies did try to capitalize on it by
marketing those little wedge pillows to make baby "more comfortable"
on the back!)

And as for co-sleeping, we know this is only something that is done
in primitive, 3rd world countries who don't have the "advantage" of
a spearate sleeping place for baby.  And, of course, we can't trust
parents not to suffocate thir babies, so we can't promote that as a
strategy to decrease SIDS.  If fact, we should ignore that factor
and actually try to scare parents away from doing it!!!!
(Please-this is intended with great sarcasm-no flames!)
