I need some advice from the many talented people out there.  I have a 7
week postpartum mom.  She has given me permission to post this question.
We are stumped and she is ready to throw in the towel.  She is hypothyroid
and hyperprolactin.


This all started approx. 4 weeks ago.  And I have tried just about
everything. She has completed the gentian violet treatment for 6 days
total, the nystatin treatment, and she also took diflucan for 8 days to try
and get rid of Thrush. She said she had some relief while using the
diflucan but she said she was never completely relieved.  He doc thought 8
days was more than enough time to be on it.  I had recommended a longer
treatment plan, until all deep breast pain was gone.  She has had red
nipples the entire time.    She tried to pump the entire day on friday and
that didn't seem to help either. In fact for some reason she was just as
sore from pumping more than when she feed him.  I have observed the baby
nursing and his latch on is great.  Not overly aggressive, but she does
have an over-active letdown.  I had her nurse on one side for 2 to 3
feeding, the switch to do the same for the other.  She said that had
helped.  She just wrote this to me today.

"This weekend I had some shooting pain deep within my breasts or when I
would
move a certain way and I was full of milk I had shooting pain.  I also
noticed that I hurt more when I am fuller.  After I feed him I feel ok but
then I just have the sore nipples that rub on my bra or t shirt or whatever.

The soreness of my nipples/aerolas is more like a rug burn/sun burn feeling
like I am just raw all the time worse when I am full.  I can't wear anything
but a bra holding me firm or a soft cotton t shirt. I can't hold him close
to me because that even hurts unless I have padding on myself.  I  really
don't know how else to describe it.  The nipples are still very pink
sometimes red.  Sometimes after I feed him I get a white discoloration at
the tips.  I have to use your nipple treatment all the time in order to form
a barrier between me and material so that it protects me. And then sometimes
when I put that on I let down and then it is washed away.  My breasts don't
know what the heck is going on.

 My breasts themselves are also sore like when you are about to get your
period and they are tender.  I can't ever lay on my stomach without a lot of
discomfort."

The baby has no sign of thrush, but we did treat baby also with gentian
violet.  I talked with her about Raynaud's syndome, but she stated cold
does not effect her and she doesn't have any other color changes.

Any insight would be great.  She has tried so hard, and is about to give
up.  Thanks
