I'd like to share that there is a support group for moms who are breastfeeding or have breastfed a child with childhood cancer (all kinds - leukemia is well-represented) online. I am the moderator. We also have on the list, and invite other, lactation professionals who are interested in collecting in one place resources such as the ones you are requesting for easy access to families who need to share it with their doctors.
