I have seen reglan induce depression in much shorter time periods, within a
couple of weeks. The mothers did not always recognize this, sometimes
figuring it out after they went off the drug. Just last month I counseled a
mother with low supply and a history of hypothyroidism and hormonal
imbalances. She had been put on reglan by her physician but I was very
concerned due to her background. I gave her the risks and benefits, and she
decided to trade the reglan in for fenugreek. A week later I followed up
with her and she said to me, "I'm SO glad that you told me about that. I'm
feeling SO MUCH BETTER now that I''m off the reglan." She went on to
describing down, depressive symptoms but she had not realized that was how
she was feeling when we first talked, not until after she went off the drug.
I have had this happen a couple other times as well. 

Reglan can be a great drug for boosting milk supply in the right situation,
but what bothers me is not knowing which mothers will benefit and which may
suffer these side-effects. It seems that postpartum mothers are more
susceptible to depression and fatigue than the avg population taking reglan.
If a mother has any personal or family history of depression or hormonal
imbalances, I worry for them and prefer other options.
