Hi Emily

Babies are also often VERY distractible at this age and don't feed 
thoroughly - if at all - some are so interested in what's going on around 
them, "sticky beaks", that mum can almost not breathe without bub coming off 
as if to say "What are you doing mum?"  Some of these babies will wake more 
at night because they are making up for it and get a lovely relaxed feed, 
mum has a great relaxed MER and all goes well for milk intake but others 
don't or mums don't allow for feeds at night. I'd also be interested if this 
pattern of feeding - i.e. four feeds in 24 hours is new or has been there 
for a while - what are poos/wees like? If baby is weeing, pooing, alert and 
healthy then plateau in weight is probably not of big concern. If 
supply/intake does seem to be down and bub will accept (and mum willing to 
offer) more feeds or longer feeds then go for it - obviously it's the higher 
preference to pumping in more solids or formula. Disappointing the "doc" 
didn't suggest extra feeds, limit solids, express if necessary etc so mum 
had the encouragement to continue almost exclusive BF and not use formula.

Couple of other questions - shorthand - is mum giving bub boiled water? this 
could be filling bub up, does bub have dummy/pacifier/thumb/fingers sucking, 
is bub allowed to cry a lot (?tired), bub medicated? mum on meds/alcohol, is 
bub actively refusing the breast?

I had a mother call me this week who I am seeing on Monday who just had
her 6 month check and the baby only gained 1 pound since then. In spekaing
with her, she feeds him only 4 times per day because he naps for long
stretches and sleeps through the night. He began cereal 2 weeks ago and
takes in varying amounts from a spoon. She reports adequate weight gain
until now (I asked for specific weights which I am waiting for from the
peds office) and the doc now wants mom to give formula 2 times per day.

I have seen many children not gain much during this time, is it out of
line to have mom give another feed or two per day at breast instead of a
bottle with formual at this point I think is a bit rash...looking forward
to seeing the mom, just wanted to make sure I was not crazy.
