I have seen nipples nearly this large...and one mom had a 4 1/2 lb SGA and 
slightly premature baby.  (Such a cruel joke of nature!)  She pumped for 3 
months with baby latching to her nipples only and gradually working her way 
to where she could take what she needed on her own.

I would warn this mom that she will likely be pumping for a long time...but 
someday the baby's mouth will grow large enough if she can keep her milk 
supply going.
