So how do you avoid choking?  While I could see how it could be effective to make the nipple protrude, my first thought was choking risk.  Also, I imagine it must be on tight enough not to slip off, but then wouldn't milk flow be restricted, mastitis risk?  If anyone could provide more details, I would appreciate it.
