Hello,
I have permission to post.  A mother that I have seen several times at our
Nursing Mothers' Group has called me with a new concern.  Let me give a
little hx.  This is her second baby - she nursed her first one for several
months with a great deal of pain and pumping.  This baby, she wanted
everything to go well from the start.  Mother and baby attended group in the
beginning for latch help - baby appeared to nurse well from the outside but
pinched mother's nipples, even with a fairly deep latch.  Her nipples became
cracked, even with a lot of assistance from the start.  Baby had a strong
suckle, high palate and mom's nipples may have been a little large for
infant's mouth.  Mother then got mastitis about a month after birth and had
a course of antibiotics and then soon after that mother got strep throat and
had another course of antibiotics.  Mother's nipple pain became much worse
although the pinching of the nipples has stopped and infant is nursing 100%
of the time (mother did have some time of pumping and only bottling when her
nipples were too sore to latch infant).  

Infant has been treated with Nystatin and mother has used Gentian Violet,
Garlic, Grape fruit seed extract and now is on her 3 week of Diflucan.  Baby
was put on Diflucan last week - infant was taken to ped and ped noted thrush
still.  (Mother called me yesterday to update me on situation - I haven't
seen her since she needed antibiotics for strep throat, but did see her at a
weight watchers meeting of all places...)  Baby is now 4 months old and the
nipples appear totally healed to the mother and there isn't an issue with
the nursing anymore except for the nipple pain she experiences during
nursing and even in between nursing - her nipples burn.  She states her
nipples do not change color, ie vasospasm.  Neither breast tissue or areola
tissue do not burn or cause any pain, just the nipples.  Mother reports pain
gets better then gets worse even while on Diflucan - I haven't not seen this
mother in person for over a month and haven't seen her nurse for a couple of
months.  I have asked to come back to the Nursing Mothers' Group, but timing
is difficult for her, but she is going to try to come to group.  Mother has
tried everything, but would like to enjoy nursing her young one.  I don't
want to miss something.

Does anyone have any ideas on this?
