I have never posted to the list (please bear with me)though I read Lacnet
daily and find it helpful! I am a new PHN and have a mother of a two week
old who was 8 lbs 7oz at birth. Infant was sent home "breastfeeding well".
By day 12 baby was not nursing as well and she began pumping after nursing
and giving infant EBM by bottle (Avent slow flow nipple). At 13 day MD appt
infant had lost a pound. MD suggested supplement with 1 oz formula after
each feeding. On day 14 I was called in and observed nursing session and
attempted an assymetrical latch. Infant does not latch deep enough despite
numerous attempts. Mother has great nipples. He became fussy after nursing
a few minutes with no swallowing heard or seen. Mother refuses to use an
SNS, finger feeder, or to take galactagogues. She is nursing the best she
can every two hours followed by pumping using the PIS until we can get her
a Lactina. She is pumping 1.5-2 0z (mostly 1.5 oz)every 2 hours with longer
time between pumping at night-total of 10 pumpings. She desperately wants
to nurse and previously nursed two other children successfully with no
supplements. I just attended a Jack Newman conference and thought of using
the feeding tube in the bottle approach hoping that this would be more
accepted than the SNS but can not find anywhere to purchase the feeding
tubes. (5fr 36inch). She is now supplementing with the EBM and formula for
total of 2-2.5 oz. I'm frustrated since she is not open to my tools. Any
ideas?

Thanks,
Lisa Bell, RN, BSN, IBCLC
