Hello:  Does anyone know of any prepared forms on the net that we out
here in the wild west call "pee &  poop sheets" or could more
technically be called "bowel movement, urination, and breastfeeding
frequency recording forms for newborns and special situations"?  I'm
making up one for my local hospital (have done this before in another
state) and find it helps to look at other people's creative layouts to
see if there are elements I want to incorporate in mine.

For anyone who hasn't used one, it helps clue moms in that
breastfeeding IS going well or that it isn't, and they need to take
appropriate action in finding appropriate guidance.  It's not the
ultimate diagnostic tool, but it's a good screen, so we can check out
those situations that need to be monitored more carefully. ("Hi, my
baby's five days old and has only stooled once." "Oh, really, why don't
you bring the baby in and we'll check to make sure he's latched on
correctly" (or whatever).  It also helps many moms to see that (for the
most part) they really aren't nursing "all the time"; you know, the mom
of a newborn who comes in angry that she's nursing all the time and when
you check her own records you find this means 6 times in 24 hours, so
she just needs a re-framing on how much care newborns need.  (And a
chance to vent a little.)

Some of the publications in my library have them as well (I just need
to look), but I wondered what current forms might be online.
