Susan,
I have had a few mothers with a history of hyperprolactinemia/infertility
and subsequent low milk supply. I have been suspicious of the use of
anti-prolactin treatments close to or even during part of the pregnancy,
though this was not a part of your scenario. How recently was thyroid
function checked? It can fluctuate during and after pregnancy, and I would
consider starting there; not just TSH and T3/T4, but also thyroid
anti-bodies. I would also ask for prolactin. Just because she has a history
of hyperprolactinemia doesn't mean that her body has responded normally to
the baby's suckling. 

Did mom have any other symptoms for her infertility? Any signs of androgens?
